Quilt Entry Form

Before starting the Quilt Entry Form gather the following information: 

1. Quilt name
2. Quilt category 
3. Names of all contributors 
4. Commercial pattern name (if applicable) 
5. Commercial pattern designer (if applicable) 
6. Width and length of quilt
7. Description of quilt (200 word limit)
8. Photograph of quilt 

Notes: 

• Quilt show rules state that there is a  maximum of 5 quilt entries per person. 
• Quilt show rules state that  a maximum of 3 quilts submitted for judging. For example:  If a person enters 5 quilts then on 3 of those quilts may be judged.
